These are all forms you'll need after you have completed Registration! If you still need to register, head to the Registration page. The forms on this page will be completed before your child starts school in the fall, and some are for reference while they're here.
Medical Statement
This is a state required form that MUST be signed by your child's physician, and returned with a copy of their immunizations list before your child can attend school. A paper copy can be obtained from the school, or printed from the link below, and returned completed to the school. If your child has any allergies or special health needs please email us, so that we can get you the additional forms you will need.
Enrollment + Health Info
This is a state required form that must be submitted by the Parent Orientation date. A paper copy can be obtained from the school, or printed by following the link below, and return completed to the school. Per the state, every single area must be filled out, even if it's just N/A.
Tuition Breakdown
If you are interested in paying the full year or 1/2 year, please email us at info.cbcp@gmail.com and we will give you information on payment methods, as only monthly payments can be made online. These payments must be made by June 1/ Orientation
